Abstract

This construct is about how we can provide optimal support of the patient and family that is facing a chronic illness. it is especially important to create a trustworthy, authentic, caring, and lasting relationship with the patient and family as they face the uncertainty that chronic illness can bring. The purpose of this poster is to explain and discuss the proper ways to provide support for the patient and family in a way that betters their overall time within our care.
